IACUC Forms
The IACUC uses eProtocol IACUC for the online submission, review and tracking of animal utilization protocols (AUP) and Animal Use Registration Forms (AURF). Refer to the eProtocol IACUC & eAnimal Ordering pages for more information.
Laboratory Self-Evaluation Checklists
The IACUC conducts semiannual facility evaluations of all the University's animal facilities, including satellite facilities. The Laboratory Self-Evaluation Checklists below contain many of the most common previous findings and labs are encouraged to utilize them when preparing for semiannual facility evaluations.

Whistleblower Policy Posting
The IACUC is obligated to investigate concerns regarding the care and treatment of animals used in research, teaching or testing at the University. Concerns about animal care and use should be reported to the Attending Veterinarian, the IACUC or University Compliance Services. The names and phone number of these contact points should be posted in animal facilities and laboratories.

Notice of Intent to Use Embryonated Eggs
Per IACUC Policy #1.3, investigators must inform the UT Austin IACUC of their intent to use live embryonated eggs of avian and other egg-laying vertebrate species for research, teaching and testing. See the policy for specific information on when this form or an animal use protocol is used, as well as additional requirements.
