This online request should be used to request animals to be transported between the ARC, DPRI, HDB and NHB. Animals will be transported within 2 working days of request to NHB and every Friday morning to DPRI. Use a Protocol Transfer form for protocol transfers between two different PI’s and then after approval you may use this Transport form to request physical transport. Are you requesting transportation for animals/cages that are being transferred from one protocol to another?
If the animals/cages you are requesting transportation for are being transferred from one protocol to another protocol, an animal transfer form must be submitted and approved prior to requesting transportation. Your PI and the person submitting the protocol transfer will receive an approval email and a corresponding approval number. Please enter that approval number below:
Cages must be clearly labeled with the destination facility and room number on every cage to be transported/transferred.
