How to Submit Your Financial Interest Disclosure (FID) Form

Federal Regulations and University policy require the completion of Financial Conflict of Interest (FCOI) training and submission of a Financial Interest Disclosure (FID) form by covered individuals.

Effective February 6, 2017, FCOI training is now housed within the FID form via the electronic submission system, eProtocol. When you complete the Policy section of the FID form and submit the completed FID form, you will have satisfied the FCOI training requirements. This means that the FCOI training module is no longer available through the University Compliance Services Compliance Training System (CTS). Prior CTS completion records will be maintained and researchers are not required to re-take the FCOI training until the record expires or when their FID form expires, whichever occurs first.)

BEFORE beginning Step 1, you will need to disable pop-up blockers or make an allowance/exception for orsc.utexas.edu. If you need help managing your pop-up settings, follow these step-by-step instructions.

  1. Use your UT EID and password to log in to eProtocol at: https://spike.orsc.utexas.edu/fid (opens in new window). You will be directed to your Home page.
  2. When you arrive at your Home page, select the “Create/Open FID” button. The FID form will appear in a pop-up window.
    NOTE: 
    If you are returning to a FID ID that you previously created and saved, you should see a created FID ID listed in preparation. Select the “Create/Open FID” button, then select “Edit” in response to the small pop-up window. The FID form will appear in a pop-up window.
  3. Begin completing your FID form in the pop-up window starting with the Policy section.
  4. Policy: Read the Handbook of Operation Procedures and mark the checkbox at the bottom of the Policy section.
  5. Use the “Next” buttons to navigate through the sections of the FID form.
  6. Research:
    • List the active and proposed University research projects that you are or will be involved in  (regardless of funding or funding source).
    • If you have a University appointment, you must include all research conducted at Seton or other local sites.
    • To manually add a project, select “click here.” Provide the Project Title and Name of Principal Investigator at minimum. If the project is or will be funded, provide the Sponsor Name, OSP #, Account #, and/or Project #, if available.
  7. Components: Respond to the Yes or No questions.
    • If you answer Yes, you must provide information about the financial interest in the sub-sections that follow after selecting “Next.”.
    • If you answer No, continue to the next question.
    • If you need to change your answer from Yes to No, you must first delete any entry or entries in the corresponding sub-section, then change your response to the Component radio button.
  8. Attestation: Read the attestation statement and mark the checkbox at the bottom of the Policy section.
  9. Check: Check the form for completeness. Return to incomplete sections as directed.
  10. Submit: Submit the FID form by selecting the blue “Submit FID Form” button.
  11. You will receive a confirmation e-mail when the FID is successfully submitted and a second e-mail when the FID is filed.

Use these instructions if you previously created and began editing a FID form, but logged out before submitting and you are now returning to resume the process.

    • If you answer Yes, you must provide information about the financial interest in the sub-sections that follow after selecting “Next.”.
    • If you answer No, continue to the next question.
