How to Update Your Financial Interest Disclosure (FID) Form

Federal Regulations and University policy requires Covered Individuals to update their FID form annually and no later than the 30th day after acquiring a new financial interest that requires disclosure. Upon receipt of the updated FID form, the Conflict of Interest (COI) Official will determine if a potential conflict of interest exists that requires a management plan. If a management plan was previously established, the COI Official will determine if the plan requires revision.

Effective February 6, 2017, FCOI training is now housed within the FID form via the electronic submission system, eProtocol. If your FCOI training is expired or will expire, you should update your FID form. When you complete the Policy section and submit the updated FID form, you will have satisfied the FCOI training requirements.

Instructions for Updating a FID Form

Use this process when your FID form expires or when you need to add or remove financial interests.

BEFORE beginning Step 1, you will need to disable pop-up blockers or make an allowance/exception for orsc.utexas.edu. If you need help managing your pop-up settings, follow these step-by-step instructions.

  1. Use your UT EID and password to log in to eProtocol at: https://spike.orsc.utexas.edu/fid/ (Opens new window)
  2. When you arrive at your Home page, select the blue “Create/Open FID” button. This will start the update process on your existing FID form.
  3. A pop-up window will appear with three options. Select the first option, “Start Update”, and the “OK” button.
    NOTE: If you start an update and do not submit, the “Start Update” option will no longer be available when you return at a later time. Follow the steps below for “How to Resume Updating Your FID.”
  4. Your FID form will appear in a pop-up window and display the Policy section. Use the “Next” and “Previous” buttons to navigate through the sections of the form.
  5. Policy: Read the policy and mark the check box at the bottom of the section.
  6. Research: Review the list of research projects.
    • Delete previously added projects that are no longer active.
    • Add new projects, as needed. To manually add a project, select “click here.” Save your addition by selecting “OK.”
  7. Components: Your previously provided answers should display.
    1. If you answer Yes, you must provide information about the financial interest in the sub-sections that follow.
    2. If you answer No, continue to the next question.
    3. If you are changing your answer from Yes to No, you must first delete the entry or entries in the corresponding sub-section, then change your response to the Component radio button.
  8. Attestation: Review the attestation statement and mark the check box.
  9. Check: The form will be checked for completeness. Return to the sections that are identified as incomplete and provide the required information.
  10. Use the “Next” button to proceed to the Submit section and select the “Submit FID Form” button. You will receive a confirmation e-mail when the FID is successfully submitted.

How to Resume Updating Your FID Form

Use these instructions if you previously started the update process and are returning to submit.

  1. Use your UT EID and password to log in to eProtocol at: https://spike.orsc.utexas.edu/fid/ (Opens new window)
  2. When you arrive at your Home page, select the blue “Create/Open FID” button. This will open the FID update that you previously started. Do NOT select the FID ID under “Accepted FIDs.”
  3. A pop-up window will appear with a choice of “Edit” or “View.” Select “Edit” in response to the pop-up window.
  4. Your FID form will appear in a pop-up window and display the Policy section. Use the “Next” and “Previous” buttons to navigate through the sections of the form.
  5. Policy: Read the policy and mark the check box at the bottom of the section.
  6. Research: Review the list of research projects.
    • Delete previously added projects that are no longer active.
    • Add new projects, as needed. To manually add a project, select “click here.” Save your addition by selecting “OK.”
  7. Components: Your previously provided answers should display.
    1. If you answer Yes, you must provide information about the financial interest in the sub-sections that follow.
